Volunteers- Instructions for Clearances and Forms

All volunteers for Meyersdale Area School District must have the following 4 clearances on file in order to be eligible to volunteer.  (In addition, Tuberculosis testing and/or a questionnaire must be completed with the school nurse)
 
The first two clearances are free for volunteers, but you must specify that you are requesting them for volunteer purposes when completing the forms.
 
Please note that depending on your level of involvement, you will still need board approval, superintendent approval or principal approval.
 
  1. Child Abuse History Clearance:
    Requests for clearance statements (now called "certifications") may be made online at https://www.compass.state.pa.us/CWIS  Click Create New Account.  Your Keystone ID will be a number you create.
  2. Pennsylvania State Police Reports:
    Pennsylvania state reports may be made online at: secured online using the state police website. Follow the directions.
  3. FBI Reports:
    Volunteers who have been a continuous resident of Pennsylvania for 10 years may complete this affidavit in place of having their fingerprints taken. Affidavit

If you need to complete the FBI fingerprint process, follow these instructions: To request FBI reports, costing approximately $25.25. Find application forms and instructions at Identogo site. use service code 1KG6ZJ.  You must Register Online to receive your ID # before going to get your fingerprints taken. Hours at the Somerset Office are Tues & Thurs. from 9:00am - 3:00pm.  Also, you must submit your registration UEID number to the MASD Administration office for final verification. 

    4. Complete the Arrest & Conviction Report

    5. Complete the TB Questionnaire and return it to Nurse Amy Rough for approval, arough@masd.net or 814-634-5123 ext. 308.
